The presence of an old acquaintance is firmly lodged in Neera's mind, and she would like your help in getting rid of it.
This quest begins the moment that Neera joins your party at 5 in Bridgefort. The Quest involves trying to get rid of the spirt of Adoy (part of her personal Quest in Baldur's Gate 1) that plagues her consciousness and concocting a potion that will rid her of him. She needs several ingredients for her potion.
The Red Wizard named Vichand at 16 will attack you if you try to take it from the chest. But he can usually be killed without drawing any hostile attention from anyone else in the Crusader Camp.
You'll need to reach 7 in the Forest of Wyrms after fighting monsters along the way. It's the entrance to the Spider Cave.
You'll have to fight plenty of spiders in the Spider Cave. The one you need to specifically kill is the Seven-Legged Spider at 6. It leaves behind the Giant Spider Leg.
You will eventually reach the Coalition Camp to start Chapter 10. Go to the tent to have a conference with the Coalition leaders at 21.
After the conference you will find Sergeant Wilch at 34 trying to bar entry to a Gnome merchant named Wilhelmina. Wilhelmina offers to sell you Wilhelmina's Miracle Turnip Juice. If Neera is in the party then she'll buy it on the spot to advance the Quest. If she isn't in the party, you can always buy it from Wilhelmina and then give it to Neera later.
It's up to you whether you persuade Sergeant Wilch to allow Wilhelmina into the Camp, and it won't really matter. Wilhelmina will disappear for good afterwards.
Now rest while Neera is in the party. She will have concocted the potion, but she now urges you to find a place that is connected to the Fugue Plane.
The party will at some point reach the surface of the Underground River in Chapter 10, starting at 1. The entrance to the Underground River is at 15. Chances are you'll have to fight your way through several groups of monsters, as well as the Crusaders guarding the gate at 12 to reach it.
You'll now be at 1. There are plenty of other Quests and encounters to be found here. But for present purposes you'll want to reach the entrance to Kanaglym at 27. You will likely have to fight groups of Myconids protecting a Shrieker at points marked 20, and groups of Dark Treants and Gargantuan Spiders led by a Corrupted Hamadryad at points marked 24.
And you'll encounter the spirit of a dragon named Halatathlaer once there. Getting past her and into Kanaglym is the start of the Ghost Dragon Guardian Quest.
You'll start off at 1 in Kanaglym. Kherriun will be at 6 as the leader of several close-by Dark Magicians. The Dark Magicians at 5 will join in as well. There is overlap in that defeating Kherriun and all of her Dark Magicians is the objective of The Cabal Quest.
Halatathlaer will initially join the fight against you, under Kherriun's control. But if you inflict enough damage on Kherriun then her soul jar, the means by which she controls Halatathlaer, will shatter. Halatathlaer's spirt will be freed and she'll instead start attacking the Dark Magicians. You can if you wish let Halatathlaer slaughter them by herself. Or you can stay and try and rack up as much xp as you can. Keep in mind that you only get xp for a given enemy if you land the killing blow instead of Halatathlaer.
She'll speak to you once the battle ends. Freeing her completes the Ghost Dragon Guardian Quest.
Now click on the pit at 5, which is actually a conduit to the Fugue Plane. Neera will have a conversation with the ghost of Adoy to complete the Quest. The party receives an 18,000xp reward.
