The quest begins once you complete the Main Quest called He Waits in Fire.
It begins in the throne room of the Kahanga Palace. Your share an audience with Queen Onekaza, along with Hazanui Karu, Captain Aeldys and Director Castol. All four of the factions, the Huana, the Vailian Trading Company, the Royal Deadfire Company and the Principi are now deeply concerned by the news that Eothas is making his way to Ukaizo. Each faction also wants to lay exclusive claim to the fabled city, and that means each faction wants your exclusive allegiance.
The Independent Path
You may decide that you want you don't want to ally with any of the factions, and want to reach Ukaizo yourself. A benefit of that path is that you won't lose any companions that have been part of your party, particularly the ones who considered themselves loyal to one of the factions (e.g., Maia, Pallegina, Tekehu). But it also requires you to build a ship that can get you through the storms of Ondra's Mortar unaided by any of the factions.
Click on the "Crafting" tab in your Inventory menu. The Blackwood Hull will be available under the "Unique" option for crafting. You'll also need to spend 64,000cp.
Make your way to #61 on the World Map. Continue northeast to sail into Ondra's Mortar. You will be asked if you want to proceed, because there's no going back to the rest of the world once you sail into Ondra's Mortar.
Sailing into the storms without a ship that is equipped with both the Blackwood Hull and the Dragonwing Sails will result in your ship failing, everyone onboard dying, and losing the game.
Even with the proper ship upgrades, you still need to make a few checks to make it through alive:
Winds will batter your ship, resulting in 10 damage to your sails.
Your Helmsman needs to be at least Expert level to get you past the Adra Pillar. Your sails take another 10 damage.
All your Deckhands need to be at least Expert level to get you past the whirlpool. Your sails take another 10 damage.
If your Dragonwing Sails are reduced to 0 health, that also means your ship will sink. So be sure you come in with a fully repaired ship before you start.
Getting through the storms with the right ship and making all the checks brings you to Ukaizo Harbor at #1. That marks the completion of the Quest by this path, as well as 50,000+xp. The Quest called The City Lost to Time starts once you arrive.
Alternatively, you can also obtain The Floating Hangman ship during the Quest called A Dance With Death. You don't need to give the ship to the Principi if you don't want to. You can sail it yourself to Ukaizo. The Floating Hangman itself is functionally a Galleon, but one where you can't upgrade either the Hull or the Sails. It also has only two compartments for spare Crew Members. However, its mists keep the storms of Ondra's Mortar at bay. So you can actually sail on through to Ukaizo without having to make any checks.
Faction Alliance
The other pathway is to get across Ondra's Mortar, but with the help of one of the four competing factions. It means sealing an exclusive alliance with one of the factions by completing the last Quest that the faction requires.
Choosing one of the factions by completing one of the aforementioned Quests will also compete this Quest as well, along with the 50,000+xp reward.
Completing the Quest by allying with one of the factions means you will not have reached Ukaizo upon completion of this Quest, unlike the Independent Path described above. Each faction has their own way of helping you reach Ukaizo during the Quest called The City Lost to Time.
