There will be a button here. You can click on it to cause spikes to kill Keherrem and the other captives in their cages at 3 if you wish. That would provide a more murderous way of completing The Missing Patrol Quest as you can loot Keherrem's Badge from his corpse. You would still need to find the key to unlock the door to his cage though.
Here are Keherrem and two other Crusaders, imprisoned within their cages. Keherrem desperately wants to be freed from his cage. Speaking to him will start The Missing Patrol Quest if you haven't already started it.
Their cages can be opened with the Jail Key after killing Ziatar at 14 for it.
A blind Priestess of Bhaal named Madele is imprisoned within this cage. She gives the Quest called The Eyeless Priestess. Her cage can also be opened with the Jail Key.
A trio of Worgs will be here.
Several Cultists will be at these spots. They leave you alone if you tell them it's your first day. They'll attack if you give them any other reply, or kill Ziatar at 14. They also tend to each have a Potion of Healing. The thieves among them carry Potions of Invisibility. There will occasionally be an archer among them that also has Arrows +1.
A Mad Cultist will be here. He'll be constantly bashing the bookcases. A Scroll of Armor and a Scroll of Blindness will be on the floor near him, which you can pick up if you wish. It's up to you if you want to kill the Mad Cultist or not. He'll leave behind a Potion of Explosions if you do kill him.
A Half-Dragon Blackguard named Ziatar will be here. She'll attack no matter what you have to say. If you fight her, all remaining Cultists at points marked 6 and the Mad Cultist at 12 will turn hostile.
This door can be opened with the Ritual Room Key, obtained by killing Ziatar at 14.
Here is a creature known as a Neothelid. It can use Dominate, Fear and Mordenkainen's Sword. It also occasionally leaves a pool of poison at its feet that can continuously damage any characters standing on it, which can encourage the use of missile weapons.
Here is Akanna, the High Priestess of the Temple. She'll attack if you encounter her before Darskhelin at 27. She'll be helped by two Aerial Servants, who are more powerful versions of Invisible Stalkers. You can lead them out of the room and attack them separately, while Akanna will stay in the room. You can then attack her afterwards.
She'll be grateful if you first killed Darskhelin before encountering her. And she'll willingly give you the Wardstone if you ask for it.
The chest behind her is locked and trapped. It contains a Scroll of Detect Illusion and random treasure. You can loot it without her going hostile if you freed her from Darskhelin's influence.
This wall has six niches. If you found the Page from Sarevok's Notes at 18 in the second level of Korlasz's Tomb, the clue it provides are the numbers 3 and 1 and 4. So the key is to click on the niche at the top-right corner, the niche at the top-left corner and then the niche at the bottom-left corner. That will open the door at 24.
This door will only open if you click on the niches at 23 in the correct sequence.
Here is the Shadow Aspect, which can be a really tough opponent. It can turn invisible and hit for quadruple backstabs multiple times. It will also use a Darkness spell whenever it can, which can Blind party members. It also has complete immunity to acid, cold, electricity and poison damage.
You can use certain cloud-type spells from off-screen, like M'Khiin's Withering Fog spell to gradually wear it down.
A Mind Flayer named Darskhelin will be here, in control of adventurers named Mohad Flintborn, Jhan Redmoons, Teth Se-Nehi and Shallen. A fight with them will be inevitable.
A Chaotic Commands spell on the lead character, or otherwise immunizing yourself against mind control, will obviously help against Darskhelin himself. He doesn't have a lot of health, so he should be the first and foremost target to take his psionic powers out of the battle. Another possibility is to use crowd control spells like Web and/or Stinking Cloud on the enslaved minions, so Darskhelin will come after you by himself and you can cut him down as quickly as possible.
Mohad Flintborn, a Dwarven Berserker, will leave behind:
