Dream and Memory is the Grieving Mother's personal quest. You can begin it by speaking to her as soon as she joins you, or any time during Acts 1 or 2.
At some point in your conversation you'll want to ask her about how you were able to enter her dreams. You can then offer to explain to her what you saw in her dreams. The correct answers involve:
A huge forest
Silver Adra
A child
Don't worry if you get any of them wrong, you'll always get another chance to get the answers right. Giving these answers is a prerequisite for the next step in the quest.
The next part of the quest triggers when you rest in any area that will not be available until Act III. You'll have another vision of Grieving Mother helping a mother give birth to a child, but this time you notice a bracelet of chimes on her wrist.
Speak to her again, and focus on the bracelet. The Grieving Mother will be reluctant to speak fully of it, other than to drop cryptic hints that it may have something to do with the Hollowborn.
There is an opportunity to suggest that she leave you for good, in which case she'll oblige. Otherwise this conversation will mark the conclusion of the quest.

"Well, now, this is a fine turn of events. I haven't led a party since...well, since Uncle Scratchy turned sick with the pox and put me in charge. He just didn't trust the dwarf, you see. Hmmn...hopefully, this time works out better than the last..." -Jan Jansen, Baldur's Gate II
