Faceless ManHere is where you’ll fight your final battle against the avatar of Akachi, the Faceless Man. Beware, this can be a grueling and tough fight. Akachi combines the powers of a Cleric, Wizard, and Fighter. This means he can heal himself, cast offensive spells, and dish out a lot of damage with his Scythe, especially if he lands a critical hit. He also deals out plenty of negative energy damage with each hit. He also has damage reduction that no weapon can penetrate. He therefore has a lot of resources at his disposal for bringing your hit points to zero.

That however is not the only way that he can kill you. He also has Spirit Eating powers that he can use on you, which will heal himself, reduce your hit points, and your Spirit Energy all at once. Of course, you can now use your own Spirit Eating powers without limit so you can trade tit for tat with him if the need arises.

It gets even better. Each time you bring his Avatar down to zero hit points, one of four aspects of his curse will manifest as a mini-avatar and he will regain some of his health. They are Remorseless Instinct, Infinite Despair, Unrelenting Fury, and Insatiable Hunger. All four of these mini-avatars must be destroyed before you can bring Akachi to final defeat.

First, make sure you’ve got the tools to survive. Bring along plenty of Heal potions or any potions that confer defensive benefits. If you have any items that protect against negative energy damage and level draining, it well behooves to equip it before entering the portal for this final confrontation. The Mask of the Betrayer and the Bracers of the Inner Planes certainly qualify. Secondly, consider the weapon of your choice. Akachi has a resistance to physical damage that no weapon can penetrate. The Silver Sword of Gith seems intended to be your final weapon, but I found it ineffectual against Akachi. What I recommend is a weapon that is enchanted to inflict at least 2 kinds of elemental damage, and if you can manage 3 then all the better. This way, you have a consistent way to reduce Akachi’s hit points, as well as those of his mini-avatars, in fairly rapid fashion. If you can defensive spells, try to get your best in right away because Akachi is going to close the distance on you very fast. After that, it’s a matter of using your best judgment as to when you can use others. If you have Epic Gate, this is worth throwing out in order to provide Akachi with potent distractions.

Once you have delivered final defeat to Akachi, you’ll get Akachi’s Scythe. You can equip your character with it and then export. You also now have one of three choices.

You can choose to use the Mask of the Betrayer to restore his identity and bring about a permanent end to the curse. Akachi’s soul is then at rest and he joins the False. You then become a famous hero who eventually returns to Crossroad Keep. Khelgar, as well as Sand if he survived, will rejoin you there.

You can choose to expel Akachi from within you. This is considered the selfish and suboptimal ending. Akachi’s hunger continues to transfer from victim to victim and the Spirit Eater curse continues to plague Rashemen. 

You can choose to devour Akachi, meaning that you absorb and become in full control of his hunger. This means that you’ll likely have to fight your remaining comrades. You end up feared across the planes, bring devastation upon Mulsantir, the Githyanki, and anyone else who slighted you in the past. Kelemvor hints that you also end up confronting the Gods themselves.

Those of your NPC companions which accompanied you to the City of Judgment will also have epilogues. 

Okku will return to slumber in his barrow, waking to later on repel a Thayan invasion.

Gann can marry you in West Harbor if you’re female and you took him up on his offer after gaining influence with him. Otherwise he becomes something of a wandering free spirit, getting bored with whatever occupies him for the moment.

Safiya has probably the most variability in her epilogues. Much depends on your alignment, whether you ended the curse, expelled it, or absorbed Akachi’s soul, and whether you accept her offer to become your romantic partner. The results can range from her trying to find a way to end the curse, to her helping you terrorize the Planes together, to both of you becoming Guardians of Kelemvor, to her taking over the Academy (either in benevolent or tyrannical fashion). If you’re good aligned and married to her, and you had recruited One of Many along the way, One of Many will devour her and eventually your PC will exact revenge.

Kaelyn also has a fair bit of variety. In order for Kaelyn to have an epilogue, you have to help her and side with the Crusade against the City of Judgment. If you helped the Crusade but Rammaq survived, he will reach a bargain with Kaelyn, only to betray her with the result of Susah and Efrem being dragged to the Abyss. Kaelyn returns to Mount Celestia a now mute and broken-spirited woman. If you helped the crusade but did not end Akachi’s curse (the suboptimal ending), Kaelyn and the Menagerie hide from the Gods in the Plane of Shadows as outcasts. Her wings turn black. If you helped the crusade and ended the curse (the good ending), she continues to lead forays against the Wall of the Faithless with her wings turning black in the process.
