My nephew was here yesterday and was playing NWN. He was playing the OC with a female Drow fighter, when I noticed that she did not have a head! Hell, I didn't even know you could create a Drow in OC or SOU. Is this common, is it just a glitch, or is it due to the fact that the boy cheats like the sun shines. :D

You can sort of create a Drow; all you have to do is call the character's race that in the sub-race bit and make the character very dark-skinned. I'm not sure if this actually makes it a Drow, but it sure creates the illusion :)
The only reason I can think of for a headless character, is if you cheat and mess around with the appearance. Another way is if you downloaded a hakpak with heads that wasn't applied to a module.
Just remember though; ointment's what you need when your head has been cut off. :D

Correct; there is no "drow" subrace, though you name yourself so. I did it in HotU, and they still called me a surfacer. And it is probably the little cheats he's put in. Too many cheats make the system go all wiry. Happened to me in BG (though the characters were not headless, they were constantly injured.)

Most commonly, it's as Lost Meme said... the module not supporting the head appearance chosen by a HAK in another module.
