occasionally, when a party member dies, he or she just dissapears from my character portraits.

these characters have NOT been turned to stone.. they have not been imprisioned or disintigrated.. they just vanish.

I just happened to me--against the twisted rune folk---aerie was killed (took PHYSICAL damage..the dialogue said 'aerie:death) after one of the twisted rune folks attacked her and appeared to do normal damage. it was not the beholder or the lich or the other mage (the pit-fiend summoning one). perhaps the vampire? not sure. I could not bring her back. not even at a temple or with a freedom scroll

this has happened to me before. has it happened to anyone else? any explanations?

It could be that you have gore turned low or off or whatever so that you don't see the chunks described above. But the reason they can't be brought back is that they were dealt an overwhelming amount of damage at once and bam...splat...boom...you get the picture ;)

Yep, this also tends to happen when dealing with firekraags firebreath - how can I resurrect my dear Aerie when I have to use a broom to collect her ashes first ... :(

I never tried this, but somewhere I spot that you can bring these companions back to life by using limited wish scroll.
    So check.
