I did not notice anything that made me think of Bhaal. I thought the guy was just a Flaming Fist soldier (considering the game is reported to use 5e rules and how FR tends to do things I would imagine that this is taking place well beyond the original game's timeline so probably not a character we would know) and the dead all looked to be other flaming fist soldiers. When thinking of Illithids and BG it reminds me of Duralg's Tower and it would be interesting if they tied them together even if it was very tenuously.

Oh, I don't remember how Bhaal is supposed to look. I suppose the tentacle face should have reminded me of Illithid first, but I don't remember them being like those things from dead space, where they spout tentacles from someone's mouth. That threw me off a bit.

If you look at the bottom video in the FAQ, it explains that.

Oh ok I get the confusion. This method of Illithid reproduction has been a thing I think since midlate 3.5 and they have kept it since. It is strange though because I think normally the tadpoles would need a host that was incapacitated for them to get in so I guess that guy must have temporarily escaped.

The tad poles are with the elder brain and then they are put in human hosts. Tadpoles that get too big are killed but if they are not they eventually grow into the giant neolithids as I recall.

You would be correct. The tadpole is deposited through one of the head's orifices into the victim's brain, where it begins to devour the brain and grow at the same time. The problem with the teaser is that ceremorphosis takes about a week to complete, but the victim's mind is destroyed in just a few hours—as soon as one of the stats is drained to zero, which happens at the rate of 1 per hour. The teaser makes it seem like this process is almost instantaneous.
