Where do I find the cup I need to wake the Eleven God avatar up?
I`ve already found the amulet and the moonblade.

the black dragon is carrying it. You will find an exit in top left corner of the map, the next area is a small clearing where the dragon is lurking :)

how you get it from the dragon is your choice

No need to turn into the slayer, you can easily take it on, without losing two of your reputation points.

If you can't beat the dragon an alternative is to drop all your decent gear before you talk to him, he takes anything you have left but still goes away leaving you with the cup

I tried to sneak invisible close to him, with the evil intention to steal that cup, but, as I said in a previous post, it seems he could detect invisibilty somehow.
Fight was "terrific": Lower Resistance+Gr. Malison+Chromatic Orb = dead meat.
I did not found a real good use to the Slayer form. It will kill you too fast, before you could do something decent. IMHO, it's not worthing.
