    I can't remember, but his might have been one of the things that gets put hautomatically into your inventory - so check there.

    I don't think it is, since one of my friends disintegrated the poor dragon that had it, and couldn't complete the game afterwards...

    Quest items are left on monsters even if you disintergrate them. I know this, because I was in the Planar Prison, and I used disintergrate on the Master of Thralls, and the only item on the floor was the orb, which is a quest item.

    I have quest item missing once or twice, I just reload and do the killing again, then everything is fine.:)
