    I'm thinking that KotOR isn't such a small game as people make out - I've spent 20 hours on it so far, and there still seems quite a long way to go. Maybe I just have a slow playing pace or something...

    Yeah, KotOR isn't very big but if you explore everything, it isn't so small, either. I hope KotOR 3 will give more incentive to revisit places than 1 and 2. If the area is already done, I don't think it's so hard and resource-consuming to throw in a couple of later-in-the-game quests with special triggers. It should actually be a cheap way to make a game longer. And I wouldn't mind that. I liked the way it was done in Might & Magic.
